Make an appointment
If you’re eligible, a DispatchHealth representative will give you a call to schedule your In-Home Health Evaluation. Or you can call us directly if you prefer. We’re available every day, including holidays.

Get information, tips and advice at home
During your visit, you can expect to spend up to an hour with our licensed medical professionals. We’ll do a head-to-toe exam, conduct relevant health screenings, and discuss your health goals. We’ll also go over the medications you’re taking and talk about your health history.

We’ll handle the rest
We’ll end the visit with practical suggestions for managing your health. If you need it, we may suggest other resources and services from your health plan. Afterwards, we’ll close the loop with your primary care doctor so your entire health care team can stay on the same page.

If your health plan says you’re eligible, the In-Home Health Evaluation is covered at no cost to you. You’ll know you’re eligible if you received a message from your health plan or if DispatchHealth already contacted you to set up an appointment.

What does an In-Home Health Evaluation cost?

If your health plan says you’re eligible, an In-Home Health Evaluation is no cost to you.

Who will come to my house for an In-Home Health Evaluation?

We employ the same experienced professionals you’d see in a doctor’s office, including physician assistants, nurse practitioners and medical technicians. Every visit is supported remotely by a board-certified doctor, if needed.

What can I expect during an In-Home Health Evaluation?

During a standard In-Home Health Evaluation, we’ll:

  • Chat about your health history.
  • Discuss how you’re feeling now and any issues you might be experiencing.
  • Take notes and keep records, flagging if there’s anything unusual.
  • Review medications you’re taking to make sure they’re right for you.
  • Give you a thorough physical exam to ensure everything’s in order.
  • Conduct preventative screenings (e.g. social determinants of health, cognitive, pain, etc.)
  • Arrange further care or assistance at home, if needed.
  • Ensure you’re up to date with important health standards.
Does an In-Home Health Evaluation replace my other doctor appointments?

No. An In-Home Health Evaluation is not meant to replace your annual wellness visit, appointments with your regular doctor, or ongoing medical care.
