We work with health plans to offer eligible members In-Home Health Evaluations — at no cost to the patient. More than just a general check-up, these visits are designed to provide an extra level of attention for your specific health concerns.

Your In-Home Health Evaluation is in addition to your regular check-up or annual wellness visit, or any other appointments with your doctor or specialist.

How it Works

In-Home Health Evaluations are available to patients who are identified and recommended to DispatchHealth by their health plan. Once we’ve received this recommendation, we’ll reach out to you to set up an appointment that fits your schedule. We’ll come to you, so there’s no need to worry about transportation, and no sitting in a waiting room. After your visit, we’ll close the loop with your usual healthcare provider so your whole care team is on the same page.

How can I request an In-Home Health Evaluation?

In-Home Health Evaluations are only available to patients recommended to DispatchHealth by their health plan; patients cannot self-refer to this service. If you believe an In-Home Health Evaluation might be helpful for you or someone you care for, we recommend you discuss this with your health insurance provider.

Is an In-Home Health Evaluation covered by my insurance?

Yes. Because In-Home Health Evaluations are only available to patients who are recommended for this service by their health plan, there is no cost to you.

Who will come to my house?

We employ the same experienced professionals you’d see in a doctor’s office, including physician assistants, nurse practitioners and medical technicians. Every visit is supported remotely by a board-certified doctor, if needed.

Why would my health plan recommend me for an In-Home Health Evaluation?
