Pre-Hospital (Acute care)
Same-day, high-acuity medical interventions in the home that provide an alternative for unnecessary/non-emergent ED visits. The DispatchHealth medical team may include an emergency medicine or internal medicine physician, physician assistant, nurse practitioner, nurse, medical technician, and/or imaging technician. They bring all the tools and technology needed to treat most non-life-threatening emergencies.
Hospital
In-home hospital alternative with 24/7 monitoring. The care team consists of a hospitalist-trained internist, supported by a team of advanced practice providers and a nurse care coordinator. Service includes daily rounding, nursing, remote patient monitoring, physical therapy, care coordination, ADL support, and meal prep.
Post-Facility
Readmission avoidance for high-risk patients via a home visit 24 - 72 hours post-discharge. Team evaluates gaps in care plan and makes the appropriate adjustments to ensure a smooth, safe transition from hospital to home. The DispatchHealth medical team may include an emergency medicine or internal medicine physician, physician assistant, nurse practitioner, nurse, medical technician, and/or imaging technician. They bring all the tools and technology needed to treat most non-life-threatening emergencies.
Follow-up visit for patients at high-risk of bounce-back within 48 hours of discharge from the ED. Medication reconciliation, condition specific assessment and care planning.
Last Mile Care
Proactive visit to high utilizers with general assessment and education - goal to reduce unnecessary use of ED.
